In EDH, a player cast Dread Summons, choosing X=3. However it was the top 3 cards that were put into my graveyard, because I had seen the top 2 from Sylvan Library.

Had this error in a commander game. I think it was caused by Asmira, Holy Avenger's end of turn trigger since it was the first turn I had play it. An opponent had cast Mystical Tutor in response to it, but I doubt that mattered. I didn't have any creatures die that turn.

Noticed in Commander against AI
When It That Betrays's annihilator activates and the enemy sac's a Door of Destinies, It That Betrays's ability makes Door of Destinies come onto the field under your control, however when it does the Select creature type popup from Door comes up blank not allowing you to pick a creature type and continue your turn.

Yeah, so my dumbass brother was using the stops wrong. Seems fine now.
