Describe the Bug:
Conversion Chamber:
1- Counter is not visible but it is there.
2- It can be activated even if tapped,(so get counter and golem same turn and again if requirements are met).
3- Creatures exiled don't go to exiled zone.
Also on another game AI tried several times but never put a counter on it and of course no golem.
Which card did behave improperly ?

Describe the Bug:
Jace, Architect of Thought and Vraska the Unseen don't get any counters or have any activated abilities. They seem to possibly have some kind of comes-into-play trigger that does nothing. No effect from legend rule either.

Yup, as you suspected the code pointer is wrong. It's fixed now.

I think this crashes the game whenever it dies. It might be because it usually dies when I get hit with a Wrath of God and I have nowhere to put the counter.

confirmed, though I only get the crash if player hit cancel, if I player target Arcbound Stinger itself, the crash will not occur.
I couldn't reproduce crash with Wrath of the God, I didn't even have the trigger if only one Arcbound Stinger is in play.

Debtors' Knell can't be cast, similar to the Creakwood Liege bug.

Describe the Bug:
I am playing Challenge 1-4 (onslaught) using the dredge deck from
viewtopic.php?f=57&t=3790#p50772
During my upkeep, I don't get the choice to return Ichorids from graveyard to play, even though conditions are satisfied. The dialog doesn't pop up.
Cannot make a savegame because of challenge mode. With the same deck, Ichorid works correctly in normal duel mode.
