When Haunted Cadaver ability trigger, game ask for sac or pass, if owner choose sac, a dupe of it is created in the gy but the original still stays in the bf. Next turn if Haunted Cadaver trigger again no other dupe is created in gy but there is no sac either.

This only happens when Haunted Cadaver was played face-down and morphed. If it is played normally it functions normally.
