Heartmender is the culprit, in your savegame after several turns and testing with Carnage Altar when Heartmender remove a counter and Carnage Altar is activated persist effect does not occur (and 255 counters show up on Heartmender.

This is also due to a global bug with Persist that has already been fixed here, but the bug with Heartmender must be different.

OK, I got the same result as stassy, but to elaborate, in the save game the next turn the creatures get 255 counters and P/T each go up by one, and each turn thereafter they lose one counter and their P/T go up by another 1 (so 2 turns after loading they have 254 counters and +2/+2). This happens to any creature with Heartmender in play that has had a -1/-1 counter on it. I tested with a Serrated Arrows on random creatures and it affects them too. So this is a Heartmender bug that goes beyond Persist.
Working on: housekeeping and archived reports

Already reported, it was a bug in the function that remove counters, it's already fixed.
