[fixed/closed]Bonehoard Germ instantly dies & others

Describe the Bug:
The Germ creature token put onto the battlefield when Bonehoard enters the battlefield instantly dies, although creature cards are in its controller's graveyard.

In the attached savegame, Bonehoard can't be equipped, but that worked in a clean game where it gave +x/+x accordingly.

Which card did behave improperly ?

What exactly should be the correct behavior/interaction ?
Bonehoard should be equipped to the Germ creature token put onto the battlefield when Bonehoard enters the battlefield. If there's at least one creature card in any of the graveyards, said token should not die on the spot.

Are any other cards possibly affected by this bug ?
Haven't checked other equipment with the living weapon ability, but they're potentially affected.

The other living weapons like Flayer Husk that also redundantly check is_equipping before setting power/toughness are also affected; the ones that don't, like Batterskull, aren't. Fixed c736fee.
