Describe the Bug:
When a creature dies while Deathbringer Thoctar is on the battlefield, a +1/+1 counter is put on the latter.

What exactly should be the correct behavior/interaction ?
When a creature dies while Deathbringer Thoctar is on the battlefield, Thoctar's controlelr may choose whether to put a +1/+1 counter on it or not.

Are any other cards possibly affected by this bug ?

Testing in a clean game I see Deathbringer Thoctar's name light up after a creature dies. Clicking it will give it a +1/+1 counter, but I am free to ignore it and it will not.

Appears to be functioning correctly.

See attached screenshot, where I have killed 2 creatures. I ignored the first trigger, and am now being prompted for the 2nd. (one creature was my own, the other was AI)

Well,it has to be bugged since I tried both savegame and clean game and it automatically gives the counter when a creature or more die.Could you please check again HarlequinCasts maybe you got confused from the Slaghter Pact triggers or something?Do you test with Duh mode off like me and Nexhro?

You can see the the screen shot that the Deathbringer Thoctar is triggering, waiting to be clicked on!

Doesn't highliight before it gets the counter in my games {duh mode=0).

I use CirothUngol's distribution patched to JOU if that makes any difference.

Weird. Here is the same situation, same two screen shots from my game. And you'll see a trigger.

The only difference I notice is that there is no rules card in yours. Otherwise I am out of ideas.

Here's the .dck files of the decks I used to reproduce the bug after it first occured, which was in a game with rules cards on both sides of the table.

As I have guessed it is a duh mode thing HarlequinCasts has it on and that causes the difference in the behaviour.duh mode is working the opposite it should on Deathbringer Thoctar as when you have duh mode off it is placing counters at will and when you have it on it is asking if you want to place counters or not.
