Still bugged in M15.

Can't cast at all without U mana available, and are never given the option for 2 life regardless.

Can be cast by paying 2 life as of CoRM15 mini2.

Prompts an unnecessary second selection popup asking whether to pay or 2 life after "Cast normally" has already been chosen from the initial popup.

Selecting "Pay 2 life for Phyrexian Mana" works normally. All modes correctly counter the targeted spell.

Additional Bug:

Mental Misstep can be used to counter cards with zero converted mana cost (for example a Mox Pearl). It should only counter cmc 1 cards.
