[fixed/closed]Thran Tome no card put into graveyard

1) When Thran Tome's activated ability resolves, its controller's library is revealed to that player's opponent.
2) That opponent gets to choose a card from among the revealed cards, but the chosen card simply remains in Tome's controller's library.
Tome's controller then draws two cards. No card is put into that player's graveyard.
Which card did behave improperly ?

What exactly should be the correct behavior/interaction ?
The top three cards are revealed rather than the whole library.
The chosen card is put into its owner's graveyard.
Are any other cards possibly affected by this bug ?
