Llanowar Reborn triggers each time a creature enters the battlefield even when it has not a counter on it.
Which card did behave improperly ?

Which update are you using?(date,name)Which type(Duel,Gauntlet,Sealed Deck)

What exactly should be the correct behavior/interaction ?
Llanowar Reborn graft ability should trigger only if it has a counter to place on creature when a creature enters the battlefield.
Are any other cards possibly affected by this bug ?
