Have you tried to use its ability more than once? I confirm that also in Constructed its ability can't be activated more than once for the whole game.

AI can still cast Man-o'-War without having to return a creature from the battlefield to his hand

Which card did behave improperly ?

Which Version did you use ?(Constructed, Limited)and which update(date,name)

What exactly should be the correct behavior/interaction ?
AI is to return a creature from the battlefield to his hand when casting Man-o'-War

Are any other cards possibly affected by this bug ?

Coat of Arms works fine. Just use the latest code pointer in Costructed version.

But Gargaroz, I was playing and I saw Mutavault 2/2 and Adaptive Automaton still 2/2 with Coat of Arms in play and plenty of creatures on my side.

That's really strange, I tried whit my Treefolk deck and everything's fine, although I probably found a small bug on Dauntless Dourbark code (it doesn't see Adaptive Automaton as Treefolk).

Mentor of the Meek should be a white card, and its activated ability should trigger when a creature with power 2 ore less comes into the battlefield under your control.

As I said abou the "saproling" bug, I forgot to update the Magic.exe part. It's fixed now, you'll find it on V3.

Adaptive Automaton seems to give +1/+1 type bonus to Mutavault but not the original copied creature.

Your savegame correctly show the bug in Tournament mode, however when playing in constructed duel Aeon Chronicler correctly adjust his stats according to cards in hand either in normal or suspended cast, weird :?
