Problems with a Couple of Cards

Just tonight I noticed a problem with how FORGE handles a couple of cards:
Ulamog, the Infinite Gyre
When Ulamog, the Infinite Gyre is cast, destroy target permanent.
I received no such prompt to destroy a permanent (it cast like any creature spell) which is kind of a bummer since I was counting on it. I didn't cast this under any unusual circumstances.
Wurmcoil Engine
When Wurmcoil Engine dies, put a 3/3 colorless Wurm artifact creature token with deathtouch and a 3/3 colorless Wurm artifact creature with lifelink onto the battlefield.
This worked, but instead of one of each (2 total creature tokens), FORGE generated 2 of each. I'm not sure if this makes a difference, but Wurmcoil Engine was put into the graveyard under somewhat unusual circumstances. I had a Wurmcoil Engine on the battlefield, as well as a Kiki-Jiki, Mirror Breaker, which says Tap: Put a token that's a copy of target nonlegendary creature you control onto the battlefield. That token has haste. Sacrifice it at the beginning of the next end step. So I tapped Kiki-Jiki, made a copy of Wurmcoil Engine, attacked with the copy of Wurmcoil Engine, and once it was sacrificed, a total of 4 tokens popped up - 2 3/3 with lifelink and 2 3/3 with deathtouch, instead of just 1 of each as the card says. The original Wurmcoil Engine was still in play, and it never went to the graveyard, so I don't know if the game handles the "real" card differently than the "token copy" or if it's just a simple fluke, or if having two Wurmcoil Engines confused it.
Mox Diamond
Tap: Add one color of any mana to your mana pool.
Occasionally, I would say roughly 30% of the time, when I tap Mox Diamond, I don't get the prompt allowing me to choose a color. Instead, it defaults to white. This also happened once with Lotus Petal, and since I couldn't undo the sacrifice, I couldn't get the correct color. Fortunately with Mox Diamond, I can click "undo" to undo the tapping, and then when I re-tap, usually I receive the prompt to choose a color. Still something worth fixing.
Karn Liberated
+4: Target player exiles a card from his or her hand.
Quite frequently, yet not all the time, I'm not given a chance to target my opponent, and I'm automatically prompted to exile a card from my own hand immediately after choosing the +4 option.
