How does the coding for winning Planeswalker Ultimates work?

Garruk, Cursed Huntsman has been broken for the last two releases. The trophy, under Planeswalker Ultimates in Achievements, cannot be earned. I have tested multiple times and met the conditions but not earned the award.

What is wrong with the following coding and how can I fix it?
