Quest Not Loading In Mac OS X Lion

Everything is working fine with Forge, but when I attempt to start the Quest mode, it begins to load, I see the "thermo bar" load to the end, then the game crashes. I get this report:

Several Mac users have had a problem while updating. They somehow lose some or all of their quest mode icon pics. Four of them reside in the /pics/ folder and 21 of them reside in the /pics/icons/ folder. If you are missing any of these, well...

I need a list of the names of all these 25 icons, & which 4 go in the /pics/ folder.

I didn't update an already existing Forge install, I downloaded the newest version.

I uncompressed the forge v1.1.1 archive and then started a new quest OK, I did not attempt to add any of my old files into the Forge.app package contents. I did not receive any errors.
