The ultimate of Vraska the Unseen does not trigger the achievement. Won a game with one of her assassin tokens but no achievement.

edit: The same goes for Tezzeret, Agent of Bolas or do you need to kill with his ultimate and not only after it with another attack?

And why is there an achievement for Teferi, Temporal Archmage when the card is not yet implemented?

Technically, Teferi has been implemented, but the set it's in hasn't been released yet, so it's not available in Beta Releases.

Nahiri's Cool Sword: "Careful, it's hot."
Nixilis's Lust for Power: "All knowledge of a lifetime... Well, not MY lifetime, but still..."
Daretti's Eternal Re-Cycle: "Reuse, recycle, repeat."
Freyalise's Regal Force: "It's easy being green."

Are there achievements for winning particular drafts in quest mode yet?

"The Original Overcosted Legend" - Legends, ofc.

"Goglgothian Sylex Operator" - Antiquities (setting that thing off caused a huge nuclear blast, made Urza a planeswalker and caused the Ice Age)

"Unicorn Gourmet Chef" for Homelands (because Feast of the Unicorn, ofc)

"Victorious Invader" should be the one for Fallen Empires (I could figure out the exact picture)

Well, Fallen Empires really doesn't have one main storyline, so maybe a collage made from the empire destroyers: Farrel's Zealot, Homarid Warrior, Thrull Champion, Orcish Captain and Thallid Devourer, maybe?

Let's have a look here...

We might start with draft of Alpha/Beta/Unlimited -- that could be called "The Original Winner". The picture could be of Black Lotus.
For Arabian Nights, "The Storyteller". The picture is Shahrazad.
Antiquities -- I'd suggest shortening the title to "The Sylex Operator" with Golgothian Sylex as picture.
Legends -- I'd suggest "The Legendary Hero". Hard to say who is the best "face" of Legends. Adun Oakenshield looks quite heroic.
The Dark -- "The Witch Hunter" would work. I think you need to keep these a bit similar, so for example they would all start with "The" and they would all be some sort of description that can be attached to a person. Witch Hunter is a good picture to use.
Fallen Empires, how about "The Empire Vanquisher"? With the collage I've mentioned before.
Now Ice Age/Alliances. How about "The Frozen One"? A nice picture to use would be Karplusan Giant.
Homelands, well, Unicorn Gourmet Chef is too specific for a single card. How about "The Home Invader"? Chandler looks like he would fit the bill.
Mirage block: "The Unphased". A picture to fit would be Spatial Binding.
Tempest block: "The Architect of Rath" was actually Yawgmoth, I'd guess. Maybe "The Ultimate Evincar"? While Volrath didn't get a card in this block, there are some cards that depict him, for example Fevered Convulsions.
Urza's block: This block is all over the place, locationwise, but it might be the best to take something from blue. I'd call the achievement "The Biographer of Urza" and used the picture from Catalog.
Masques block: This block was also strange, story-wise, as each of the three sets happened on different plane, and all happened roughly at the same time. But Rebels were in the whole block, so I'd call the achievement "The Leader of Rebellion" and used picture of Cho-Manno, Revolutionary.
Invasion block: "Space Invader" doesn't fit much. "The Planar Invader", how about that? And the poster child is, of course, Tsabo Tavoc.
Oddysey block: "The Threshold of Madness" is not really a title for a person, but maybe we can use Chainer's picture and give the title "The Dementia Master".
Onslaught block: Morph was one of parts of this block, but not really the main thing. That was tribal theme. I'd suggest "The Chief of the Tribe". The picture of Voice of the Woods would fit.
Mirrodin block: I don't think they used the term "Mirran" back then, but "The Ravager of Mirrodin" would sound at least as good. But I kinda think it would be nice if the title pointed at the "main" expansion, and Arcbound Ravager is from Darksteel (maybe there could be actually different achievements for whether you play with one, two or three sets?). So maybe, for Mirrodin, "The Connoisseur of Metal" with picture of Gilded Lotus, to mirror the Unlimited achievement.
Kamigawa block: with the Japanese theme, I'd simply call it "The Shogun". For picture, I'd suggest Takeno.
Ravnica block: "The Guildmaster" is perfect But which of the guildmasters to use? From the four in Ravnica I'd guess Szadek would be the best.
Coldsnap: You didn't mention this one, but Coldsnap was its own draft environment. The title might be "The Lord of Ice", with picture of Heidar, Rimewind Master.
Time Spiral block: I'd suggest "The Temporal Archmage", Teferi's new moniker in Commander 2014, but picture of Teferi, Mage of Zhalfir. After all, why not give him a title from another time?
Lorwyn block: It was a tribal block and important part of that were the changelings. "The Tribe Uniter" with picture of Mirror Entity.
Shadowmoor block: The block was about color and it actually did contain a five-colored card. "The Pumpkin King" with picture of Reaper King.
Alara block: "The Maelstrom Conqueror" with picture of Child of Alara would be fun, but I wanted something from Shards of Alara. If colors are unequal, my default color to go is blue (as it best represents the player), so maybe "The Shard Conqueror" with picture of Sharding Sphinx.
Zendikar/Worldwake: "The Intrepid Explorer". Cliff Threader would be a nice picture.
Rise of the Eldrazi: "Leveler" doesn't really work as a title. Perhaps "The Experienced Explorer". A leveler that would fit that description might be Skywatcher Adept.
Scars of Mirrodin block: "The Metal Overlord". Picture would be of Platinum Emperion.
Innistrad/Dark Ascension: "The Monster Hunter". Picture of Elite Inquisitor.
Avacyn Restored: "The Restorer of Avacyn" with Avacyn, Angel of Hope.
Return to Ravnica block: This is a complex one since each of the set had different draft environment. This at least requires an achievement for every set.
RTR/RTR/RTR draft: "The Genius of Guilds" with Niv-Mizzet, Dracogenius.
GTC/GTC/GTC draft: "The Crasher of the Gates" with Aurelia, the Warleader.
DGM/GTC/RTR draft: "The Runner of the Maze" with Exava, Rakdos Blood Witch.
Theros block: "The Classical Hero" with Fabled Hero.
Khans of Tarkir/Fate Reforged: "The Clan Leader" with Surrak Dragonclaw.

Yep, those are all rather good, even though tripple Cold Snap isn't something I'd encourage (and I would encurage Ice Age / All / CSP though). I kinda went more with iconic single card / pun titles and general puns on how achievements in steam games are named / set related memes - but I wouldn't mind seeing it done like you did it, it's a pretty damned flawless list!

And I know "The threshold to madness" isn't a title, but it's a fine pun and you do need to hit it to be good at drafting that particular block XD I actually wrote that first before the "title" theme got going. I'd go with "Dementia Master" with Chainer as the picture like you say, though (funny story, my nickname used to be Chainer back in the day because of a hilarious local mispronouncing of the word which made it sound like a nickname of some Yougoslavian street-level thug from a particular part of Yougoslavia. We used it as a short name for the card "Chainer's Edict" and I played them often, and when you said it it sounded like you got mugged by that guy - "There's Čeeyner commin at ya", so the nickname stuck. Due to the edict not the actual Chainer card (which in line with the other pun we used to call "Chainer, demented mobster") ^^)

And if we're going full cannon story here, for Fallen Empires the only real deal is "The Doom of Fools" with Thevers Szat on the picture - but the damned clown isn't mentioned in the set itself and the only pictures of him are in Invasion block. A collague as you suggest is a good deal, though.

"Unphased" for Mirage is impossibly perfect.

A few more notes on Marek's list:

Ice Age/Alliances - Necropotence is the definite face of this block, the name does need a bit of thinking since "The Potent Necromancer" is all I can come up with.

"Home invader" I like for Homelands, the picture does need to be Ishan's Shade most likely. Either that or Baron Sengir.

Tempest block: "The Ultimate Evincar", yep there's plenty of him in the art all over the place, but Black cards tend to be the most obvious choices. Maybe "The Stronghold Buster" with some Eladamri art?

Well, "The Pumpkin King" is a reference to Nightmare Before Christmas, and Reaper King has a rotten pumpkin for head in the art.

I thought of re-using Explorer because it's still Zendikar, with "Experienced" to be a hint that he leveled up, with a picture of a leveler.

How about "The Hunter of the Night" then? That can be taken in two ways, either you are hunter who works at night (monster) or you hunt the night.

Or in Rise of the Eldrazi: "Have Chandra Ablaze, Jace, the Mind Sculptor and Sarkhan the Mad on the battlefield and cast a colorless Eldrazi card." (since these three were the ones, IIRC, who opened the lock and released the Eldrazi).

I like this idea of "story achievements" a lot. I'd say its worth having it's own category for if we can come up with some others.

Sadly, with all the refactoring and initial network stuff I've been working on recently, I haven't had time to add new achievements or even investigate a couple of the broken Planeswalker ones. I would definitely encourage anyone interested in helping out to go in and try to make the changes themselves. The code should be pretty easy to work with.

The key is utilizing player.getAchievementsTracker() for tracking any information necessary during the game (if it's not already information available via other game/player properties), then using that information at the end of the game in an Achivement override class to determine whether the achievement has been earned (and what tier if applicable).

Well, story achievements can be naturally separated based on how many cards you need for them.

Some more ideas:
Sacrifice Vhati il-Dal to Commander Greven il-Vec.
Alternately, you could kill Vhati il-Dal with Predator, Flagship. Giving him flying with its first ability is optional

Use Skyship Weatherlight to put anyone from the Weatherlight crew into your hand. Weatherlight crew was unfortunately split into many sets, but it should include: Captain Sisay, Crovax the Cursed (not Ascendant Evincar!), Gerrard Capashen, Ertai, Wizard Adept (not Ertai, the Corrupted!), Hanna, Ship's Navigator, Karn, Silver Golem, Mirri, Cat Warrior, Orim, Samite Healer, Squee, Goblin Nabob and Tahngarth, Talruum Hero. I guess that Legacy Weapon should also count. There could be a separate achievement if you pull the "alternate" crew (Crovax, Ascendant Hero or Mirri the Cursed).

Get rid of Xenagos, God of Revels using Deicide OR exile him via Godsend.

A possible "alternate win" achievement could be to win a game with Marit Lage, as that powerful eldrazi-like entity needs first to be freed from Dark Depths , requiring you to work toward that goal as it takes lots of mana to achieve this.
