How to create and connect to a distant serveur ?

Hey guys, as many of us noticed, serveurs are a bit unstable recently. I wandered if it is possible to connect directly from a computer to an other distant one, so that i can play with a friend. I already successfully did it in local, but i want to do that with two computers that have different IPs (not connected on the same box). How ?
