Collectible Card Game Headquarters

Describe the Bug:
Orcish Mine when the last counter is removed it triggers 2times dealing 2+2 damage.Probably the 1st triggers as soon as the counter is removed and 2nd later as the land gets destroyed.

What exactly should be the correct behavior/interaction?
Orcish Mine triggers only once.

Are any other cards possibly affected by this bug?

Also doesn't play nice with planeswalkers. In my test AI had one when I cast Orcish Mine. When the Orcish Mine triggered it asked if I wanted to do 2 damage to the planeswalker, and when that didn't kill it the 2nd 2 damage remained on screen but unapplied to anything until more damage was done to AI.

The bug seems to happen if there is also another trigger in upkeep that has to do with counters. The bug doesn't happen if there are no other triggers.
