I cast Inquisition of Kozilek and I got this assertion fail error:

and then a runtime error and then crash:

Works in a clean game. If Death Wish brings it into players hand in your other save it works. Don't know...

But I'll confirm off your errors.

I don't think this can be replicated now and in limited test it worked fine so this is now closed.
