Describe the Bug:
If a divinity counter attached to a permanent created from That Which Was Taken is present in the bf when Sway of the Stars is cast, it will stay in the bf after resolution.
Which card did behave improperly ?

I confirm the bug and probably that is the case for all Legacy effects.
What is interesting is that the effect attached itself to a black Lotus I cast the turn after I played Sway of the Stars.

That's strange, as code an values seems fine. What exactly happens, did Spell Crumple go on the bottom of your deck ?

Yes, as you cast it, the countered spell still process and Spell Crumple go to the bottom of library

AI can't get it has to assign at least 3 blockers to block Phyrexian Colossus and don't get the damage.

AI gets that if he doesn't assign 3 blockers Phyrexian Colossus will still do assign damage to player.

confirmed but only on combat damage, if opponent is dealt spell damage Carnage Wurm will trigger his Bloodthirst ability.
I tested it in a clean game, but with your save Bloodthirst seems to be not working at all, must be another interaction or effect carried over in Tournament mode.

confirmed though I think it's quite hard to code, on AI side it happen the same but the defending player can manually choose to assign 3+ blockers so the combat damage are not dealt to him (if he choose to assign less than 3 blockers damage go through as if the attacking creature had trample).

It's really strange I made another game and Carnage Wurm worked fine with an instant( lightning Bolt ) and with combat damage.
It can't be a sealed tour problem as other cards with bloodthirst in the same tour worked fine.It has to be another interaction but can't find what.

Rusted Relic is not a creature when I control 3 or more artifacts. In my test, I controlled 8 artifacts before Rusted Relic entered the battlefield. I have not tested having 2 artifacts, then adding a third to see if it switches at that point.

I can confirm it but seems to be an issue with artifact land as using non artifact land will trigger Rusted Relic.
Also when using artifact lands, if you try to cast Rusted Relic with them mana will be put back into the mana pool once (casting Rusting Relic again will work but it won't trigger to artifact count, probably broken at that step)

You're using "trigger" strangely. Rusted Relic doesn't have any activated or triggered abilities. Becoming a creature isn't put on the stack here. If its controller controls 3 or more artifacts, Rusted Relic is meant to be an artifact creature as part of that game state.
I guess regarding the computer game, Rusted Relic's controller controlling 3 or more artifacts should be among the checks any time the game state changes, which is probably how Mox Opal works, which isn't bugged.

I am not using the technical vocabulary from mtg rule codex but the ML game engine one, which still use the old rules and "programming" states, pardon my french 
Anyway it's an interaction issue between artifact lands and Rusted Relic, not an issue with the number of artifact in play.
